Informed Consent  for Outpatients individual Psychotherapy services

The following information concerns your rights and responsibilities as a client.

 

Services

Individual therapy aims to alleviate psychological distress through talking in one to one meetings with a qualified mental health provider rather than through the use of medication. Individual therapy has been found to be helpful for a wide variety of problems including relational difficulties, work conflicts, dealing with a painful loss or death, stress, anxiety, and depression.


Records

Your treatment records will be securely maintained from the time of treatment until 1 year after treatment ends when they will be destroyed. Clinical records are not released to clients as they contain information that can be misunderstood by a non- healthcare professional. A client may however request treatment information to be released to another healthcare professional for a fee.

 

Confidentiality

In general, the privacy of all communications between a client and a psychologist is protected by law. Treatment information is therefore only released to third parties with client’s written permission. The exceptions to confidentiality are when a child is in danger, you (the client) are at risk of hurting yourself or another person, or when clinical records are subpoenaed by the court. In the rare event of any of these situations, confidential information disclosed will be limited to the minimum necessary.

Professional Fees

There is a fee for the psychotherapy services as advertised on this site, which may change from time to time. Payment is to be made at the beginning of every appointment on the site via electronic means.

Cancellation Policy

Please provide at least 24 hours notice if you are unable to attend a scheduled session.

 

Client Consent

By agreeing to this consent, you acknowledge that you freely consent to receive psychotherapy services the assigned doctor and you understand your rights and responsibilities as a client.
